A blended average across attendees works well for a rough figure: add up each person's fully loaded hourly cost (salary plus benefits and overhead, not just take-home pay) and divide by headcount.

No, only the meeting itself. If prep, notes or follow-up tasks take meaningful time, add that to the duration to get a fuller picture.
